The item is a brand new set of motor mounts.  The picture is accurate, match it to the one you need.  This mount meets or exceeds OE spec and fits the following:

• 2006-2007 Dodge Ram 1500 2500 3500 5.7L Engine ( 2 Wheel Drive Only )
  
• Please note because the same casting is used for both of these mounts the rubber on both mounts says "R" even though one is left and one is right
