Juracek was last seen August 31 at approximately 4:30 p. m. at 120 Kingsway in Belleville. According to police, Juracek was last seen driving a white 2013 Honda Accord 4 door with an Illinois license plate number of 9369037. Police believe Juracek suffers from dementia and has suicidal tendencies.
